It covers this website only. Once your child is our patient, the health information in our records is protected by federal and state law, and our Notice of Privacy Practices explains your rights. You can ask us for a copy at any time.
Information you give us
When you send the request form, we receive the name of the parent or guardian, a phone number, an email address if you give one, the child's name and date of birth, the name of the insurance plan, the services you are interested in and your preferred language. If you use the chat on this site, we receive the messages you write.
Information collected automatically
Like most websites, ours records technical data such as the internet address of your device and the time of your visit, to keep the site secure and to limit abuse of the form. We use only the cookies the site needs to work, such as the one that remembers your language. We do not use advertising or tracking cookies.
How we use the information
To contact you about your request, to verify the insurance coverage you tell us about, to schedule an evaluation, and to answer your questions. We do not use it for advertising and we do not sell it.
Who we share it with
With the insurance plan you name, to verify benefits. With the companies that host and maintain our systems, which must keep the information confidential and may use it only to provide their service to us. With authorities when the law requires it. With no one else.
The chat on this site
The chat may be answered first by an automated assistant and then by a member of our team. Please do not write medical details in the chat. The assistant does not give medical advice. In an emergency, call 911.
Phone calls, text messages and email
When you check the box on the form, you agree that we may contact you by phone, text message or email about your request. You can ask us to stop at any time.
Children's privacy
This website is for parents, guardians and health professionals. We do not ask children to give us information through it.
How long we keep it and how we protect it
We keep the information from a request for as long as we need it to respond and as the law requires. We limit who can see it and we protect our systems against unauthorized access. No website can promise absolute security.
Your choices
You can ask us what information from your request we hold, ask us to correct it, or ask us to delete a request that did not become a patient record. Contact us by phone or email.
